SHARC does collect age, name, genders, emails, and phone numbers, home addresses, and personal demographics when intake forms are created.
SHARC collects information through the website contact forms and intake forms.
SHARC uses the personal information proved to contact the patient for the following: Upcoming appointments, appointment changes, insurance updates, appointment modifications, post appointment summaries, and so on. SHARC values your time and will only contact you to move the appointment process further faster.
As stated above, SHARC is HIPPA compliant, and all information gathered is saved in an encrypted Electronic Health Records system
SHARC will never share or sell the phone numbers collected for the SMS consent with third parties or affiliates under any circumstances.
The patient has rights to access, updated, or delete their information that has been shared.
